What is a 3D/4D Ultrasound?

3D ultrasound shows a three-dimensional picture of your baby, and 4D gives the added dimension of time, allowing you to see your baby’s movements. 3D/4D ultrasound is NOT used for diagnostic purposes. It is used to promote the bonding experience between mother, father and baby, and allow siblings and grandparents to share a sense of closeness with your baby. The optimum time to schedule this 3D keepsake ultrasound is 30 weeks. It may be helpful to drink plenty of water for a few days before your appointment to ensure plenty of clear amniotic fluid, enhancing baby’s image.

 

Southern Crescent Women’s Healthcare will make every effort to get the best possible images of the baby. We CANNOT promise you that the images will be similar to those you may have seen elsewhere. Every ultrasound is different based on the gestational age, position, amount of fluid, placental location (or location of body parts that may interfere with views of the face or other body parts). While we always try our best, we cannot guarantee that we will always meet everyone’s expectations.

Keepsake 3D/4D Ultrasound Consent Form

You will be required to sign a consent form on the day your Keepsake Ultrasound is performed.

I understand that 3D/4D ultrasound technology does not replace my diagnostic ultrasound done between 18 to 20 weeks. As a patient of Southern Crescent Women’s Healthcare, this diagnostic ultrasound will be scheduled for me at the appropriate interval.

I understand that image quality may vary depending on my size and weight, the baby’s position, placenta location and amount of amniotic fluid. Southern Crescent Women’s Healthcare cannot guarantee that they’ll be able to obtain an ideal image of my baby.

I agree to pay for services prior to the ultrasound being performed. I have read and understand the above information. I understand that I am to have an elective ultrasound for a keepsake portrait of my baby, and this ultrasound is not for diagnostic purposes.
